Do you have to take part?

If you choose not to allow your patient information to be used as part of the Population Health programme, this will be respected.

You will only need to opt out of the Population Health programme via the West Suffolk NHS Foundation Trust. We will ensure that none of your health or care data is included as part of this programme.

Choosing to opt-out will not affect how you access any clinical services. You will continue to receive health and care when you need it.

By opting in you will benefit from:

  • your health and care practitioners being able to make more effective decisions relating to your care
  • quicker diagnosis and treatment
  • more accurate prescriptions
  • less repetition and paperwork, and
  • fewer unnecessary tests.

If you opt out and then later change your mind and want to opt back in, contact us at info.gov@wsh.nhs.uk and we will ensure your records are included in the programme.

Making a choice on behalf of your children

Parents or legal guardians can opt in or opt out on behalf of children under the age of 13.

Making a choice on behalf of another individual

You can opt in or opt out on behalf of another person who is unable to do so independently. You must have a legal authority to do this. For example, a power of attorney. Please complete the electronic form indicating you are the patient’s representative. We may require proof of your legal authority.
