You're choosing to opt-out
This opt-out form is solely for the purposes of the Population Health programme (HealtheIntent).
Choosing to opt out won’t affect how you access any clinical services - you will continue to receive health and care when you need it. If you opt out we will ensure that your health and care data is not included as part of this programme.
To opt out you will need:
- to be aged 13 or over, and
- your NHS number and the postcode of your GP surgery. Your NHS number is a 10-digit number, eg 943 476 5919. You can find this on any letter sent to you by the NHS, on a prescription, or by logging in to a GP practice online service.
Parents or legal guardians may opt out on behalf of children under the age of 13.
If you have legal authority to do so (e.g. a power of attorney) you may opt out on behalf of another person, or opt back in on their behalf. We may ask you for written proof of such an authority.
