Community cardiac rehabilitation

Cardiac Rehabilitation has been proven to speed up recovery and significantly reduce future heart problems.

The programme is facilitated by a specially trained team who will support and advise you on risk factor management and exercise enabling you to regain your confidence and return to your everyday activities.

Introduction to Cardiac Rehabilitation

Services provided

Initially you will have a Cardiac Nurse led assessments, with identification of risk factors, medication review, setting of personal goals.  Our Cardiac Nurses will complete a full cardiac assessment and advise you on risk factor management.  Throughout your programme we will support and advise you.  We encourage the involvement of family and friends to provide education and support to you all.

When ready we will progress you to the exercise phase of your programme enabling you to regain your confidence and return to your everyday activities.  At this time you will meet our Cardiac Specialist Exercise Instructor who will prescribe a programme tailored to meet your individual needs and goals. 

We have multiple types of programmes available which are either virtual, home or group based:

Home-based Programmes are facilitated through your Cardiac Rehabilitation team.  You will be supported through this programme with frequent telephone contact.

Group-based Programmes (virtual groups also available) are circuit based and although set in a group environment everyone works to their own individual intensity.

We run heart information sessions once every 12 weeks, which you will be expected to attend to one of these during your time with us.  There is also opportunity for you and your family to learn CPR – live saving skills with our team.

Monthly Mindfulness sessions with Wellbeing Suffolk are available and we would encourage you to attend one of these sessions.

Inclusion criteria

You will be included in our cardiac rehabilitation if you have been diagnosed with any of the following in the past three months:

  • ACS: STEMI / NSTEMI / PCi / PPCi / Stable Angina / SCAD
  • Cardiac Surgery: CABG / Valve Surgery / Other Cardiothoracic
  • Stable Heart Failure & Cardiomyopathies
  • Congenital
  • Heart Transplant
  • ICD / LVAD AND CRT
